Physical Examination

Students registering for the following activities at Buffalo High School must have a current physical examination (dated after May 1, 2024) uploaded to the current 2024-25 registration profile before the student is authorized to participate in practice and/or competition:

  • Golf
  • Cross Country
  • Volleyball
  • Football
  • Swimming & Diving
  • Wrestling
  • Basketball
  • Soccer
  • Track & Field
  • Marching Band
  • Spirit Squad

To ensure clearance for participation, completed physical forms are required to be uploaded using this online registration prior to the first day of practice.
